The Federal University of Health Sciences, Azare shall open their admission portal for candidates with requisite qualifications to apply for FUHSA 2024 Post UTME/DE. There will also be subjects to be written and / or oral examination as will be determine by the University’s management. According to the Federal University of Health Sciences Azare, admission is open to all Nigerians irrespective of ethnic group, creed, religion, gender and disability. REQUIREMENTS FOR FUHSA 2024 POST UTME/DE
Applicants for FUHSA 2024 Direct Entry (200 level) must take note of the followings;
- Direct Entry applicants must obtain at least two (2) A-Level (or its equivalent) passes in relevant subjects, or possess credit pass in National Diploma (ND), or a good Bachelor’s degree (First Degree) in another related field as the case may be.
- There shall be screening interview by the University for those who meets the admission requirements.
Applicants for FUHSA 2024 POST UTME must take note of the followings;
- All admissions into the university shall be through the Joint Admissions and Matriculation Board (JAMB).
- Applicants must obtained at least five (5) credit passes on their O-level result (WAEC, NECO, or equivalent) in relevant subjects including Mathematics and English Language at not more than two sittings.

- There shall be screening interview by the University for those who meets the admission pre-requisite requirements.
- Currently the Federal University of Health Sciences, Azare does not accept transfer students since the institution is still in her infant stage.
- Post UTME applicants must meet the general admission requirements and also satisfy the faculty and departmental requirements.
General requirements for both FUHSA 2024 POST UTME AND DIRECT ENTRY.
- Applicants must obtained at least five (5) credit passes on their O-level result (WAEC, NECO, or equivalent) in relevant subjects including Mathematics and English Language at not more than two sittings.
- They must upload their O-level certificate on JAMB portal. Furthermore, Direct Entry applicants upload their O-level certificate alongside their A-Level, or ND, or BSc.
- Both FUHSA 2024 Post UTME and Direct Entry applicants must be at least 16 years of age before applying.
- Screening application fee is N2000 for both FUHSA 2024 Post UTME and Direct Entry.
- Applicants must use an active email address and a valid phone number during the registration.
METHOD OF APPLICATION
The application process FUHSA 2024 Post UTME/DE is ONLINE. Use the steps below for a hitch free registration process.
- First, click here to visit the Federal University of Health Sciences, Azare website or click here to visit the FUHSA 2024 POST UTME/DE application portal at once.
- If you are using the first option click on PORTALS on the upper right of the school website homepage. Then select ADMISSION which is the first option on the drop box. More so, if you are using the second option it will take you directly to the Federal University of Health Sciences, Azare application portal.
- On the portal page click on the NEW APPLICATION Enter your NAME, and a valid PHONE NUMBER. Furthermore, ensure that the phone number use for the application belongs to you for it will be used in subsequent communication with you. You will be directed to a login page.
- Login using your PHONE NUMBER as your username and also password. When you login successfully, it will take you to the application welcome page.
- On this stage you are to confirm the information here and then proceed to select a payment method. You can make your payment online with your debit card or print out your payment invoice, then take it to make your payment at any commercial bank branch of your choice.
- After payment return to the application portal and then fill your application form. Enter your personal information which include your names and other relevant information. Use the same phone number you used during account creation.
STAGE TWO
- Select your choice of program from the list of programs of studies available on the portal.
- Add your EDUCATIONAL QUALIFICATIONS. Only one or two sitting(s) O-level is accepted. Also, Direct Entry applicants are to enter their last institution, class of diploma / statement of result obtained (A-level, ND and BSC.). Details of your O-level result like centre number, and then examination number of all results you have entered will be required.
- UPLOAD YOUR PASSPORT PHOTOGRAPH. Here, you are to upload a recent scanned passport photograph with a clear and plain background from your PC. Lastly, the picture must be JPEG format and not more than 50kb in size.
- Preview your entries to avoid mistakes as these may lead to disqualification. Therefore, all information must be checked to make sure they are correct especially to avoid spelling mistakes.
- Then, click on the SUBMIT APPLICATION button to complete your application. Note that without clicking the submission button your application is incomplete.
- Lastly, click on PRINT APPLICATION button to print a hardcopy of the application form or acknowledgement slip. Also take note of your SERIAL NUMBER on your slip as this will act as your APPLICATION ID. Finally, take note for your login username and password.
AT SCREENING CENTRES
Applicants are required to attend the screening in persons before their application can be process. Applicants are required to present the originals of the following credentials:
- O-Level Certificate(s)
- JAMB original result with candidate’s photograph on it for Post UTME applicants. While registration slip is for Direct Entry applicants.
- A-level Result, Diploma, BSc or IJMB as applicable to you. This is for Direct Entry applicants only.
- Certificate of Birth / Declaration of Age.
- Certificate of Origin
- A copy of printed FUHSA 2024 Post UTME/DE application form or acknowledgement slip. This must be printed in colour for clear view.
CONCLUSION
Photocopies of the above listed documents shall be submitted at the screening centre. In addition, they will be accompanied by a utility card (scratch card(s) for the checking and confirmation of the submitted O-level result(s) from the examination body(s) (WEAC, NECO etc.). Finally, applicants for both FUHSA 2024 Post UTME / DE will be contacted via SMS on their phone and email provided for their date, time and venue for their screening.
